Make it happen

Once the evidence reviews were completed, work packages turned their attention to identifying which best practices to test and in which healthcare settings and geographical locations.  More information will become available here as this work progresses.

Diagnosis and post-diagnostic support

This work package is  constructing pilot projects on three main topics:

  • a destigmatisation programme
  • a nurse/GP cooperation programme
  • a telemedicine programme to improve dementia detection in nursing homes.

The implementation of the 3 pilot projects began in January 2018. The final phase of work will focus on post-diagnostic support for people living with dementia.

Crisis and Care Coordination

This work package will now agree which best practice models for change should be tested and in which localities testing will take place to improve care quality for people with dementia in residential settings.

Residential Care

This work package will now agree which best practice models for change should be tested and in which localities testing will take place to improve care quality for people with dementia in residential settings.

Dementia Friendly Communities

This work package will now:

•    pilot test and refine the model proposed to ensure the input of people with dementia throughout;

•    ensure that the pilot sites cover large and small populations, be in a mix of urban and rural settings, be spread across a number of countries with different cultures, languages, economic circumstances and health and social care systems; and they should also be a mix of fledgling and more mature dementia friendly communities.

•    to maximise synergies with the Dementia Friendly Initiatives framework and toolkit being developed by the World Health Organisation
